I want to report a doctor, but don't know the proper way of doing it.?

First off, I know of this doctor that's over-prescribing narcotic medicine to some elderly people. I wanted to know how to go about reporting this doctor. I've been told to report this doctor to the Medical Board of Examiners, but don't know how about going to do it. I want to remain anonymous while doing it. Thank you.
